Volunteer Application

Individuals wishing to become a Hospice Northwest volunteer must first submit an application,  either online (below) or on paper. For a paper copy, please click here. 

After the Volunteer Coordinator has reviewed the application, selected applicants will be contacted to set up an interview. 

Applicants for volunteer positions will also be required to submit 2 personal references.  Please ensure that you print off the REFERENCES FORM after you complete the online application form. The references must be received by the Volunteer Coordinator prior to being accepted into the Training Program and need to be sent into the office ASAP after submitting your application. 

 

Please Note: A criminal records check will be requested prior to placement with a Hospice Northwest client.
